Sagarmatha Choudhary Eye Hospital (SCEH), started as a small eye unit 25 years ago, today is a high volume treatment centre, which provides high quality comprehensive eye services at a cost affordable to a huge population in eastern Nepal and northern India.

In 2007 SCEH and its satellite centres had to cope with a tense political situation especially in South-East Nepal and a total of 136 transportation or general strikes ("Bandhas"). Patients could not travel to and from the hospital. In spite of these difficulties SCEH and its satellite centres could still keep up its high numbers in patients examined and operated.

Biratnagar Eye Hospital (BEH), started on 13th September 2006 as a new branch of SCEH Lahan, has successfully completed its first year of services. Under the leadership of the Medical Director Dr. Sanjay Kumar Singh it has progressed rapidly into an eye hospital widely recognized for delivering affordable high quality eye care services to the economically poor and under-privileged people of the community.

Dr. Singh started a Cornea Department in co-operation with Nepal Eye Bank, Tilganga Eye Centre and transplanted already 33 corneas in 2007.

In addition a Cornea Collection Centre and awareness programmes for Eye Donation were started in mid-November 2007 in co-operation with Tilganga Eye Centre and the Rotary Club of Biratnagar.

Special Services at SCEH and BEH

Paediatric Department at SCEH and BEH:
8,262 Children were examined at SCEH. 1,313 underwent surgery at both centres, out of which 829 were on cataract.

Vitreo-Retinal Department at SCEH and BEH:
The Vitreo-Retinal Departments of SCEH and BEH are very well equipped with up-to-date technology.

Dr. Iris Winter, CBM Ophthalmologist and Vitreo-Retinal specialist from Germany, who had joined SCEH in October 2006, moved the surgical VR unit from Lahan to Biratnagar in June 2007 and opened an own VR Department at BEH. 16,626 patients were examined and 626 vitreo-retinal surgeries performed at SCEH and BEH.

At SCEH Lahan medical retina services continued. 1,158 retinal laser procedures and 7,483 B-Scan Ultrasound examinations were performed at both centres.

To make these services affordable, vitreo-retinal surgery including endolaser, silicon oil etc. is provided for only NPR 2,500 (USD 39).

Glaucoma Department:
A Glaucoma Department was established at SCEH in April. 2,365 patients underwent special glaucoma investigation.

2,862 computerised visual field examinations were done at both centres.

Corneal Ulcer Department at SCEH and BEH:
1,886 new patients with corneal ulcer received treatment according to the lab findings.

Low Vision Department at SCEH and BEH:
753 patients with low vision were assessed, including 237 children. They received special low vision devices and counselling.

Subsidy to poor patients:
95% of all patients pay a total of NPR 900-1,300 (USD 14-20) for major surgery.
7,170 poor patients presenting at the SCEH, BEH or in the Screening Camps, mainly from Nepal, received subsidized or free treatment or even food and transportation support, with the centres subsidizing the total of NPR 1,868,758.50 (USD 29,200).

Outreach:

Satellite Centres:

- Malangwa:
14,500 outpatients were examined. 1,535 eye surgeries were performed by a Lahan ophthalmologist visiting once a week.

- Rajbiraj:
17,230 outpatients were seen and treated or referred to SCEH.

- Siraha:
5,023 outpatients were seen and treated or referred to SCEH.

- Primary Eye Care Centre in Diktel, Khotang District:
3,430 patients were examined and treated or referred to Lahan if necessary in this remote hill district of Sagarmatha Zone.

Cataract Screening Camps:
105 Screening Camps were performed in Lahan, Biratnagar and Malangwa in collaboration with local organisations in the 6 Terai districts Siraha, Saptari, Udayapur, Morang, Sunsari and Sarlahi. Out of 19,654 patients examined 2,912 were transported to SCEH Lahan, Biratnagar or Malangwa and underwent cataract surgery subsidised or free of cost.

School Screening & Paediatric Camps:
3,324 children were screened in 16 schools for eye defects and refractive errors and 14 children referred to SCEH Lahan for special refraction. 35 children found with Vitamin A deficiency received Vitamin A capsules.

In a special paediatric camp, among 346 examined children 14 were found with refractive errors.

    Human Resource Development and Training:

    Besides continuous medical education for the centre's ophthalmologists the following special teaching and training programs were performed:

    Anterior Segment Fellowship:
    Three additional Ophthalmologists from India joined SCEH and one joined BEH for the two-year Anterior Segment Fellowship, which also includes training in sutureless cataract surgery (Fishhook Technique) and phaco.


    Co-operation with BP Koirala Lions Centre for Ophthalmic Studies (BPKLCOS), Kathmandu and BP Koirala Institute of Health Sciences (BPKIHS), Dharan:

    Residents: 8

    4 Residents from BPKIHS, Dharan

    and 4 Residents from BPKLCOS, Kathmandu attended a one-month
    comprehensive clinical and surgical training.
    SCEH is co-sponsoring three candidates for the three-year Residency
    in Ophthalmology at BPKLCOS Kathmandu.

    Optometrists: 4
    4 Optometrists students from BPKLCOS KTM underwent a two-months training.

    Ophthalmic Assistant (OA) Training:
    Two OA Trainees are still under their three-years training.

    1-Year Ophthalmic Training for Medical Officer:
    Dr. Hari Sharma completed his one-year ophthalmic training at SCEH and is joining the three-year Residency in Ophthalmology at BPKLOCS since April 2007.
    Dr. Amar Keyal joined SCEH in February. He will apply for Residency Programme in Ophthalmology at BPKLCOS Kathmandu in 2008.

      Various other Trainings:
      - One-day regional Low Vision Meeting on 14.1.07, organised by NNJS National Low Vision Programme, 7 participants from several eye centres in Nepal
      - One-month clinical posting for 4 German medical students in March
      - 6-months Training for Primary Eye Health Workers at SCEH and BEH:
      - Completion of Training for 26 candidates by 31.3.2007.
      - Completion of training for 11 candidates from 2.4.07-30.9.07
      - start of training from 1.10.07 for 21 candidates
      - 14 candidates in 2 batches at BEH (7.1.-6.7.07 and 15.6.07.-14.1.08)

      - Short-term Primary Eye Care Training of 15 days for 39 Staff Nurses (3rd year) in two batches from 28th May to 26th June 2007

      - Mr. Renjith Krishnan, Fellow in Eye Hospital Management from Aravind Eye Hospital, Madurai: 6 weeks external posting at SCEH from 16.5.-30.6.07

      - Mr. Shiv Charan Mehta (BEH) underwent a one-month Eye Bank Technician Training at Nepal Eye Bank, Tilganga Eye Centre, Kathmandu.

      - Mr. Arun Chaudhary (BEH) underwent a one-month Instrument Maintenance Training programme at Himalayan Eye Hospital, Pokhara.

      - A four-member team from the Comprehensive Community Based Rehabilitation Tanzania (CCBRT), a CBM project in Dar-es-Salaam/Tanzania visited BEH and SCEH for a one week orientation in August

      - In return Dr. Sanjay Singh (BEH), Mr. Bindeshwar Mahato (SCEH) and Mr. M.S. Dhakal (NNJS) visited the CCBRT centre during the last week of September for orientation and also participated in a short-term Project Cycle Management course.

      Miscellaneous:

      Staff at the Base Hospital SCEH and BEH:
      The 189 member staff consists of 11 ophthalmologists (1 VR surgeon, 3 long-term, 7 under 2-years Anterior Segment Fellowship) and 1 Medical Officer, 10 administrative staff, 20 ophthalmic assistants and 2 ophthalmic assistant trainees, 58 eye health workers and 27 eye health worker trainees, 60 others (watchmen & security guards, maintenance helpers, gardener, drivers, sweepers, peons etc.). These figures include 49 staff working at BEH.

      "Cataract Package Tours" to SCEH Lahan:
      28 organizations in North India are operating on their own initiative "Cataract Package Tours" to Lahan for a fixed rate. This includes screening people for cataract, transport and food for the patient and one attendant as well as the fee for surgery.

      Throughout 2007, 347 busses and 13 jeeps brought 12,785 patients to Lahan for cataract surgery.

      Project Visitors:
      28 visitors came to Lahan mainly to observe the eye work.

      Co-operation with Impact Nepal:
      In view of the high prevalence of hearing impairment and ear diseases, SCEH and Impact Nepal entered a Memorandum of Understanding in September. SCEH is providing part of its premises for a Primary Ear Care Centre (PECC). A surgical team of Impact Kathmandu came to Lahan and performed ear surgeries in October.

      New Agreement for Eastern Regional Eye Care (EREC) Programme (2008 - 2012):

      In April 2007 an independent team, comprising of an ophthalmologist, a chartered accountant, representatives of the Social Welfare Council (SWC), Ministry of Health & Population and Nepal Netra Jyoti Sangh (NNJS), evaluated the project's operation and management performance during the past five years.

      A special Team of Competence, headed by CBM Co-Worker Mr. Michael Schueber, helped with the preparation of the new agreement and planning of the EREC Programme.

      The Agreement between SWC, CBM and NNJS for another five years was signed in December. The aim is that within the next five years the two centres Sagarmatha Choudhary and Biratnagar Eye Hospital will develop into the Eastern Regional Eye Care Programme, providing comprehensive eye care and tertiary eye services to the entire region.

      CBM assigned Mr. Josef Schneider from Austria to work as Project Manager for the new EREC Programme in Biratnagar. He and his family arrived in Nepal in mid-October 2007 for a four-year period.

      The construction of new Eastern Regional Eye Care Centre facilities will start in 2008.

      SUMMARY:

      In spite of the difficult political situation, the year 2007 was again very successful with nearly 55,000 eye surgeries performed, including many operations on children and patients with vitreo-retinal diseases. This high number as a result of the excellent eye services and their affordability has a remarkable impact on improvement of the quality of live and economic situation of reversibly blind persons and those at risk of blindness. To perform nearly 55,000 eye operations with only 11 ophthalmologists at two eye centres required excellent management, high motivation, very hard working and a good team spirit.

      Acknowledgements:

      The Programme Director gratefully acknowledges the support rendered by CBM and their donors and the partner organizations Ministry of Health, Social Welfare Council and Nepal Netra Jyoti Sangh and the local community. He also thanks Tilganga Eye Centre in Kathmandu for providing eyes for cornea transplants from its Eye Bank and for the co-operation in establishing an own cornea collection centre at Biratnagar Eye Hospital.
