Again a successful year

Throughout the last 24 years the Sagarmatha Choudhary Eye Hospital (SCEH) has grown from a small eye unit to a high volume treatment centre, which provides high quality comprehensive eye services at an affordable cost to a huge population in eastern Nepal and northern India.

HIGHLIGHT No. 1: Start of Biratnagar Eye Hospital

During the first 3 ½ months of its operation
8,789 patients examined,
1,645 surgeries performed,
1,441 being cataract operations
including 35% phaco surgery.

Dr. Sanjay Singh also established a Cornea Department and performed 7 cornea transplantations.

Biratnagar Eye Hospital

[Biratnagar Eye Hospital]

Biratnagar, the second largest city in Nepal, 125 km east of Lahan, has a good infrastructure and all the facilities which Lahan is lacking. Therefore it is much more attractive for ophthalmologists. To overcome the constant shortage of long-term ophthalmologists at SCEH in Lahan and to balance SCEH's enormous patient load, Biratnagar was chosen as location to establish a new eye hospital.

The very experienced Nepali Ophthalmologist Dr. Sanjay Kumar Singh, with special interest in cornea and community ophthalmology, joined SCEH in May in preparation for his services as the Medical Director of the new Biratnagar Eye Hospital (BEH), which started on 13th September. It is anticipated that in future Biratnagar Eye Hospital will develop into the Eastern Regional Eye Centre, and that ophthalmologists working at BEH could help to overcome the manpower shortage of SCEH.

Special Services

Paediatric Department:
At SCEH and BEH 9,215 Children were examined. 1,693 underwent surgery, out of which 880 were on cataract.

Vitreo-Retinal Department:
The Vitreo-Retinal Department of SCEH is very well equipped with up-to-date technology.
Dr. Chirag Bhatt completed his two-year assignment as Vitreo-Retinal specialist in May. Dr. Iris Winter, CBM Ophthalmologist and Vitreo-Retinal specialist from Germany, joined SCEH in October and took over from her German CBM colleague Dr. Bernd Fassbender who completed his two-year assignment in December. The work done in the Vitreo-Retinal Department has been still increasing with 995 vitreo-retinal surgeries and 1,290 retinal laser procedures performed.
To make these services affordable, vitreo-retinal surgery including endolaser, silicon oil etc. is provided for only NPR 2,500 (USD 36).

Corneal Ulcer Department:
2,224 new patients with corneal ulcer received treatment according to the lab findings.

Low Vision Department:
1,022 patients with low vision were assessed, including 267 children. They received special low vision devices and counselling.

Services to incurable blind:
Counselling services were provided at the base hospital to 414 blind persons, including 452 children.

Subsidy to poor patients:
95% of all patients pay a total of NPR 900-1,100 (USD 13-16) for major surgery.
6,387 poor patients presenting at the SCEH or in the Screening Camps, mainly from Nepal, received subsidized or free treatment or even food and transportation support, with SCEH subsidizing the total of NPR 2,026,887 (USD 28,956).

Outreach:

Malangwa:
12,888 outpatients were examined. A Lahan ophthalmologist visiting once a week performed 1,952 eye surgeries.

Rajbiraj:
17,525 outpatients were seen and treated or referred to SCEH.

Siraha:
6,202 outpatients were seen and treated or referred to SCEH.

Primary Eye Care Centre in Diktel, Khotang District:
In this Primary Eye Care Centre (PECC) in the remote hill district of Sagarmatha Zone 2,495 patients were examined and treated or referred to Lahan if necessary.
The annual Surgical Eye Camp was organized in the District Hospital of Diktel from 29th to 31st October. During these three days, Dr. Bernd Fassbender, a senior ophthalmologist of SCEH, and a SCEH team treated 721 patients and performed 72 operations, mostly on cataract

Cataract Screening Camps:
SCEH conducted 124 Screening Camps in collaboration with local organisations in the 6 Terai districts Siraha, Saptari, Udayapur, Morang, Sunsari and Sarlahi. Out of 25,403 patients examined 2,547 were transported to the SCEH Lahan or Biratnagar or Malangwa and underwent cataract surgery subsidised or free of cost.

School Screening& Paediatric Camps:
3,626 children were screened in 19 schools for eye defects and refractive errors and 17 children referred to SCEH Lahan for special refraction. 52 children found with Vitamin A deficiency received Vitamin A capsules.

According to the findings of the school screening, 8 villages were surveyed for children with Vitamin A deficiency.

Five paediatric camps were performed. Among 3,607 examined children 3 were found with cataract, 44 with refractive errors and 31 with Vitamin A Deficiency.

"Cataract Package Tours" to SCEH Lahan:
25 organizations in North India are operating "Cataract Package Tours" to Lahan for a fixed rate. This includes screening people for cataract, transport and food for the patient and one attendant as well as the fee for surgery.
Throughout 2006, 353 busses and 13 jeeps brought 10,854 patients to Lahan for cataract surgery.

Teaching and Training:

Besides continuous medical education for the centre's ophthalmologists the following special teaching and training programs were performed:

Visiting Faculty:
- Prof. Christian Mardin from the Erlangen University in Germany provided clinical training to ophthalmologists, residents and ophthalmic assistants in March 06.
- Prof. Arin Chatterjee from India stayed again in Lahan from 7.5. - 5.6.06 and provided a one-month refresher training for ophthalmologists and ophthalmic assistants.

Anterior Segment Fellowship:
Three additional Ophthalmologists from India joined SCEH for the two-year Anterior Segment Fellowship, which includes training in sutureless cataract surgery (Fishhook Technique) and phaco.

Co-operation with BP Koirala Lions Centre for Ophthalmic Studies (BPKLCOS), Kathmandu and BP Koirala Institute of Health Sciences (BPKIHS), Dharan:

  • 2 Residents from BPKIHS, Dharan, attended each a one-month comprehensive clinical and surgical training.
  • 4 Residents from BPKLCOS, Kathmandu attended a one-month comprehensive clinical and surgical training, 4 residents a two-month training, supervised by the BPKLCOS Faculty Dr. Jyoti Baba Shrestha.
  • SCEH is co-sponsoring Dr. Lila Puri from Nepal for his three-year Residency in Ophthalmology at BPKLCOS Kathmandu since 28th April 2005 and Dr. Pratap Karki since April 2006.

    Optometrists: 5
    5 Optometrists students from BPKLCOS KTM underwent a two-month training, the first batch being accompanied for one week by an Instructor.

Ophthalmic Assistant (OA) Training:
One OA trainee completed his training and two OA Trainees are still under their three-year training.

1-Year Ophthalmic Training for Medical Officer:
Dr. Hari Sharma joined SCEH on 15.12.2005 and will apply for Residency Programme in Ophthalmology at BPKLCOS Kathmandu in 2007. In 2006 he learned all basics in eye examination and treatment and performed already 1,500 cataract surgeries, including more than 1,000 with the sutureless Fishhook Technique.

Short-term Surgical Training:
CBM Co-worker Dr. Manfred Mörchen attended a six-week surgical training in preparation for his assignment in Cambodia.

Various other Trainings:
- 6-months Training for Primary Eye Health Workers at SCEH:

- Completion of Training for 19 candidates by 31.3.2006 (1st batch).
- Training for 4 candidates from 1.4.-30.9.06 (2nd batch).

- start of training for 26 candidates (3rd batch) from 1.10.06.

- Mr. Sudhir Kumar Thakur participated in "Management Priorities Training in Eye Care" at Aravind's LAICO institute, 15.-21.1.2006.

- Mr. Dipendra Chaudhary (7.-9.5.06) and Mr. Samit Kumar Chaudhary (11.-13.5.06) participated in Clinical Low Vision Refresher training at Himalaya Eye Hospital, Pokhara.

- Short-term training in orthoptics/paediatrics conducted by Ms Aparajita Manoranjan from Nepal Eye Hospital KTM: 11.-17.5.06

- Mr. Ram Sewak Sah participated in Certificate Course on FFA and USG at Aravind Eye Hospital, Madurai from 1.6.-28.7.06.

- Mr. Satya Narayan Singh went to Bangalore for Low Vision clinical TOT, Low Vision Trainer of Trainer Workshop, Bangalore – 3.-7.6.06.

- Mr. Bimal Poudyal and Mr. Durgesh Raj Pant, NNJS KTM conducted a one day Low Vision Interaction Programme on 4th July for 9 participants from CBR/Special School teachers.

Conferences, Workshops and Courses:
DOC (Congress of German Ophthalmologists) in Nürnberg/Germany, 23.-26.6.05: Dr. A. Hennig: 1. Cataract Surgery in Nepal, Changes in two decades: From ICCE with spectacles to phaco; 2. Model for the training of ophthalmologists on the sample of Nepal; 3. Long-term Assignments in Developing Countries; 4. Seminar on Eye Work in Developing Countries; 5. Wetlab Instruction Course on Sutureless non-phaco Cataract Surgery (Fishhook Technique)

CBM Planning Workshop for Biratnagar Eye Hospital
Mr. Rolf Müggenburg, Continental Director Asia from CBM Headquarters, Germany visited Nepal twice in preparation for the new Biratnagar Eye Hospital, first in January, then again:
From 3.-6.12.06 a CBM Planning Workshop was held in Biratnagar for developing a strategic programme concept for Biratnagar Eye Hospital transforming into the Eastern Regional Eye Centre, facilitated by Mr. Michael Schueber, Organisation Development Advisor to SCEH and BEH.

Miscellaneous:

Staff at the Base Hospital SCEH:
The 108 member staff consists of 7 ophthalmologists (1 VR surgeon, 2 long-term, 4 under 2-years Anterior Segment Fellowship) and 1 Medical Officer, 5 administrative staff, 15 ophthalmic assistants and 2 ophthalmic assistant trainees and 42 eye workers, 36 others (watchmen & security guards, maintenance helpers, gardener, drivers, sweepers, peons etc.). One VR surgeon left SCEH in May and one long-term ophthalmologist end of December.
Biratnagar Eye Hospital has 27 staff working under Dr. Sanjay Singh.

Hospital Management Information System and Data Collection:
The hospital is equipped with computers in the Administration, Outpatient Department and Operation Theatre, which are partly linked by Networking and connected to a Data Backup Server.
Also, a special Hospital Management Information System (HMIS) was designed in-house according to the hospital's requirements and implemented since 2005. Data on book keeping / accounts, inventory management, billing as well as patient record data including pre-op, post-op and VA findings are now easily available for effective decision-making, quality control as well as for statistical analysis, reporting and research. This system was introduced also at the new Biratnagar Eye Hospital from the beginning.

Project Visitors:
- CBM delegated the CBM Co-worker and Organisation Development Advisor Mr. Michael Schueber, for 2 1/2 months from 8th March and again for nearly two months from 31st October, especially to move forward with setting up the new Satellite Hospital in Biratnagar and to facilitate a CBM Planning Workshop for Biratnagar Eye Hospital.
- Dr. Anil Kumar Gunda was sent from Aravind Eye Hospital, Madurai for one month from 19.2.-17.3.06 to help during the surgical peak season.
- Dr. Robert Blum, a young Medical Doctor from Australia, interested in Community Ophthalmology, worked at SCEH for 3 weeks from 11.-28.10.06 - 42 visitors came to Lahan mainly to observe the eye work.

Venerable Ceremony:
On 4.1.06 the Lahan Chamber of Commerce & Industries, Lahan, Siraha organized a Venerable Ceremony for Dr. A. & Kristina Hennig

First Film Prize:
During the video competition at the German Congress of Ophthalmic Surgeons (DOC) in June Dr. A. Hennig's film on "Cataract Operations in Nepal, Change in two decades: From ICCE with spectacles to Phaco" won the First Prize.

SUMMARY:

The year 2006 was very successful with nearly 60,000 eye surgeries performed, including increasing operations not only on cataract patients, but also on children and patients with vitreo-retinal diseases. This high number indicates the excellent results and affordability of the eye services. Many patients sometimes travel two to three days to Lahan for treatment.

To perform nearly 60,000 eye operations with only eight ophthalmologists required excellent management, high motivation, very hard working and a good team spirit.
