The comprehensive eye services of the Sagarmatha Choudhary Eye Hospital (SCEH) in Lahan were established and consolidated with the support of Christoffel Blindenmission (CBM) in co-operation with the Nepali partners Social Welfare Council (SWC) and Nepal Netra Jyoti Sangh (NNJS). The present five-year agreement was renewed with the Ministry of Health, the SWC, NNJS and CBM.

Throughout the last 22 years the SCEH has grown from a small eye unit to a large volume treatment centre, which provides high quality eye services at an affordable cost to a huge population in eastern Nepal and northern India.

Surgery:

A Phaco-Vitrectomy unit (OERTLI OS-3) and a ZEISS Operating Microscope (Visu 160) with BIOM/inverter and camera were added.

Cataract Surgery:
48,824 cataract operations were performed in 2004. Sutureless Fishhook Technique was with 44,228 the most commonly used procedure. Phaco surgery increased to 989.

"Cataract Package Tours" to SCEH Lahan:
13 organizations in North India have started "Cataract Package Tours" to Lahan for a fixed rate. This includes screening people for cataract, transport and food for the patient and one attendant as well as the fee for surgery.
Throughout 2004, 214 busses brought 6,797 cataract patients to Lahan for surgery.

Special Services at SCEH, Lahan

A TOPCON Fundus Camera and a Pulsair Tonometer were added as special equipment.

Vitreo-Retinal Department:
The Vitreo-Retinal specialist Dr. Chirag Bhatt joined SCEH end of May 2004. With his support 458 retinal surgeries/procedures were performed in 2004.

Paediatric Department:
8,074 Children were examined. 720 underwent surgery, out of which 565 were cataract surgeries with 96% IOL implantation.

Corneal Ulcer Department:
1,640 patients with corneal ulcer received treatment according to the lab findings.

Low Vision Department:
265 patients with low vision were assessed, including 65 children, and received special low vision devices and counselling.

Services to incurable blind:
Counselling services have been provided at the base hospital to 685 blind persons, including 79 children.

Satellite Centres:
In all Satellite Eye Centres patients are seen and treated or referred to Lahan by experienced Ophthalmic Assistants.

In Rajbiraj a Lahan ophthalmologist is visiting for special treatment and recently started with minor surgery.

At the Satellite Eye Hospital in Malangwa, once a week a Lahan ophthalmologist performs major surgeries (2004: 1,631).

Establishment of a Primary Eye Care Centre in Diktel, Khotang District:
To reach the patients in the remote hill district of Sagarmatha Zone a Primary Eye Care Centre (PECC) was established in Diktel of Khotang District, starting with a surgical eye camp within the Diktel District Hospital from 18.-22. April. During these five days, 1,428 patients were examined and treated and 164 eye operations performed, mostly on cataract.
Since then, continuous outpatient services are provided by one of the Lahan Ophthalmic Assistants along with a trained local Eye Worker.

Cataract Screening Camps:
SCEH conducted 99 Screening Camps in collaboration with local organisations in the 6 districts Siraha, Saptari, Udayapur, Sunsari, Morang and Sarlahi as well as in Khotang district. Patients found with cataract were transported to the SCEH or the Satellite Hospital in Malangwa and underwent cataract surgery subsidised or free of cost.

Subsidy to poor patients:
95% of all patients pay a total of NPR 900-1,100 (USD 12.70-15.60) for major surgeries.
2,550 poor patients presenting at the SCEH or in the Screening Camps, mainly from Nepal, received subsidized or free treatment.

School Screening & Xerophthalmia Surveillance Programme:
3,032 children were screened in 23 schools for eye defects and 32 were referred to SCEH Lahan for special refraction and receiving glasses. 40 children with Vitamin A deficiency received Vitamin A capsules.
According to the findings of the school screening, 21 villages were surveyed and another 66 children with Vitamin A deficiency found and supplied with Vitamin A capsules.

Teaching and Training:

Besides continuous medical education for the centre's ophthalmologists the following special teaching and training programs were performed:

Ophthalmic Residency Program:
5 residents from BP Koirala Institute of Health Sciences, Dharan attended a one-month comprehensive clinical and surgical training.

Sutureless ECCE/IOL Training:
4 ophthalmologists from various countries received short- or long-term training in sutureless cataract surgery (Fishhook Technique).

Ophthalmic Assistant Training:
After a three-months theoretical training at Ran-Ambika Lumbini Eye Hospital, Bhairahawa 7 candidates are undergoing a three-years practical ophthalmic assistant training at SCEH Lahan.

Various other Trainings:
· Prof. Christian Mardin from the Erlangen University in Germany provided clinical training to ophthalmologists, residents and ophthalmic assistants.
He was also the main speaker during the Eastern Regional Workshop at Lahan.

· Prof. Arin Chatterjee from India provided a one-month refresher training for ophthalmologists and ophthalmic assistants.

· 2 batches of 20 Nurses from Unique Educational Academy Rajbiraj received a two-weeks training at SCEH.

· 15 paramedicals are undergoing a six-months training at SCEH Lahan.

· 17 Traditional Healers received a three-days training in primary eye care in collaboration with HMG Udayapur district hospital.

· 2 OT staff of Ramlal Golchha Eye Hospital, Biratnagar visited SCEH Lahan twice for short-term training.

· 2 Ophthalmic Assistants of SCEH received a short-term Low Vision refresher training in Pokhara and Lahan.

Miscellaneous:

Staff:
The 119 member staff consists of 9 ophthalmologists (1 VR surgeon, 5 long-term, 3 under 2-year Anterior Segment Fellowship), 7 administrative staff, 13 ophthalmic assistants and 7 ophthalmic assistant trainees, 47 eye workers and 36 others (watchmen, maintenance helpers, gardener, drivers, sweepers, peons etc.).
Dr. Jitendra Kumar left the hospital in August after more than 6 ½ years of service.

Support to Ramlal Golchha Eye Hospital (RLGEH) in Biratnagar:
A two-year Memorandum of Understanding was signed between RLGEH and SCEH for strengthening the co-operation between the two centres and to promote the eye activities at the RLGEH. Staff of RLGEH visited SCEH for orientation and training.
SCEH helped to conduct 8 cataract screening camps in the eastern region, and a Lahan team performed 213 sutureless cataract operations at RLGEH.

Project Visitors:
71 visitors came to Lahan to observe the eye work.
On request of CBM Mr. Michael Schueber spent six weeks in Nepal to assess the Human Resource Management and Development situation of SCEH, specially in view of long-term sustainability.

Awards:
In March 2004, Dr. A. Hennig received an "Outstanding Humanitarian Service Award" and the "Certificate of Appreciation" by Nepal Netra Jyoti Sangh regarding "the development and expansion of eye care services in the eastern part of Nepal and outstanding service and humanitarian work to fight blindness in Nepal over two decades."
